Initiating Your Electronic Chart


Wellspring Midwifery Care uses electronic charting, so it is requested that you complete your initial intake forms electronically prior to your first appointment.


In order to start your chart, you must click on the link below and fill out the intake forms. This will create your electronic chart on the practice's iPad and save you the time of completing your background information and history at your first appointment. Be sure to click submit when you finish!
Please visit the following link from an iPad, Mac, or Windows PC. The iPad is preferred as it allows you to review and sign the consent forms and agreements:
Please fill out as much of this form as possible before your first visit. This will give us more time to get to know each other at your first visit.

If you are unable to open the client portal from your Mac or Windows computer, please follow these steps:
  1. Close all your browsers and try the link again.
  2. Reboot computer and try the link again.
  3. Try a different browser or computer if available.
  4. Even better, if you have access to an iPad please open the link I provided from there.
  5. Enable Silverlight plugin :
    • Safari - Be sure the Safari->"Private Browsing" menu item is unchecked. Then open menu item Safari->Preferences->Security and press "Manage Website Settings...". Then choose Silverlight on the left and ensure "When visiting other websites" is set to "Allow Always".
    • Firefox - Open the “Tools->Add-ons” menu. Press “Plugins” on the left and make sure the Silverlight plugin is set to “Always Active”.
    • Internet Explorer - First be sure there is nothing that says “InPrivate” at the top by the web address. If so, restart your browser. Then press the gear icon in the upper right-hand corner and choose “Manage add-ons”. Then choose “All add-ons” under the “Show:” field. Then scroll down on the right and make sure the “Microsoft Silverlight” add-on is “Enabled”.

As a midwifery client, the Texas Department of State Health Services wants to ensure you are given certain information during the course of your care. These documents and brochures can be found on this page. Please download the documents and read them prior to your appointment. 

Medical Records Request

We may request previous pregnancy records (for example, if you had a previous surgical birth or are transferring care from another provider during the current pregnancy)

In order to request records from your previous provider, please download the following form to request your records be faxed to Wellspring Midwifery Care.

Insurance Coverage

Questions about health insurance coverage? Wellspring is out-of-network, and offers in-house billing for a small fee. All clients are expected to pay in full by 36 weeks (either the full package cost or your portion after expected insurance coverage). See the Cost for Services page for more information. Please contact us for further details.
