Wellspring Midwifery Care uses electronic charting, so it is requested that you complete your initial intake forms electronically prior to your first appointment.
As a midwifery client, the Texas Department of State Health Services wants to ensure you are given certain information during the course of your care. These documents and brochures can be found on this page. Please download the documents and read them prior to your appointment.
We may request previous pregnancy records (for example, if you had a previous surgical birth or are transferring care from another provider during the current pregnancy)
In order to request records from your previous provider, please download the following form to request your records be faxed to Wellspring Midwifery Care.
Questions about health insurance coverage? Wellspring is out-of-network, and offers in-house billing for a small fee. All clients are expected to pay in full by 36 weeks (either the full package cost or your portion after expected insurance coverage). See the Cost for Services page for more information.
